If you are a United States Resident and were sent notice by Defendant that your Private Information was potentially impacted in the Data Incident, you may be a Class Member and eligible for Settlement Benefits. Excluded from the Settlement Class are: (1) Defendant, any entity in which Defendant has a controlling interest, and Defendant’s senior executive management, successors, subsidiaries, and assigns; (2) governmental entities; and (3) the Judge assigned to the Action, that Judge’s immediate family, and Court staff.
If you are a Settlement Class Member and wish to receive a pro rata Cash Payment, then you must submit a claim form by June 3, 2026.
To start your online claim form, you must login with your Unique ID and PIN. You can find your Unique ID and PIN on the Notice that was sent to you.

The deadline to submit a Claim Form online is June 3, 2026. If you are mailing your Claim Form, it must be mailed with a postmark date no later than June 3, 2026.
